Piles vs Fissure vs Fistula — What's the Difference?

Piles, fissure, and fistula are three distinct anorectal conditions that are often confused with each other — but they have different causes, symptoms, and treatments.

Piles (Hemorrhoids)

Piles are swollen blood vessels inside or around the rectum and anus. They are graded 1 to 4 based on severity.

*Symptoms:* Bright red bleeding after bowel movements, itching, discomfort, a lump near the anus.

*Treatment:* Grade 1–2 often respond to medications and dietary changes. Grade 3–4 require MIPH (stapled procedure) or conventional hemorrhoidectomy.

Anal Fissure

A fissure is a small tear or crack in the lining of the anal canal. It causes intense pain during and after bowel movements.

*Symptoms:* Sharp pain during defecation, bright red blood on toilet paper, a small skin tag near the fissure.

*Treatment:* Acute fissures may respond to medications. Chronic fissures need lateral internal sphincterotomy (LIS) — a small, precise surgical procedure.

Anal Fistula

A fistula is an abnormal tunnel connecting the inside of the anal canal to the skin near the anus. It usually develops from a prior abscess.

*Symptoms:* Recurrent pain and swelling, pus or discharge near the anus, fever, difficulty sitting.

*Treatment:* Fistulotomy, seton procedure, or other surgical techniques depending on the tract's location and complexity.
